* Wallet passphrase lock fixes:
- wallet should not allow cli commands to bypass lock validation
- wallet should lock again in mixing mode if it was unlocked for mixing and next unlock (e.g. for sending funds) happened
- removed AnonymizeOnlyUnlocked status, it's never going to be reached anymore
- "For anonymization only" checkbox:
- should be available only when user click "Start Mixing"
- should be set on by default when available
- adjusted title
- adjust error messages in walletpassphrase for unlocked wallets

QT_NO_KEYWORDS prevents Qt from defining the `foreach`, `signals`,
`slots` and `emit` macros.
Avoid overlap between Qt macros and boost - for example #undef hackiness
in #6421.
- remove default value for parent variable in dialogs, which should appear
centered over main GUI and pass appropriate value
- add some addition NULL pointer checks
Use misc methods of avoiding unnecesary header includes.
Replace int typedefs with int##_t from stdint.h.
Replace PRI64[xdu] with PRI[xdu]64 from inttypes.h.
Normalize QT_VERSION ifs where possible.
Resolve some indirect dependencies as direct ones.
Remove extern declarations from .cpp files.
- try to enforce the same style to all Qt related files
- remove unneeded includes from the files
- add missing Q_OBJECT, QT_BEGIN_NAMESPACE / QT_END_NAMESPACE
- prepares for a pull-req to include Qt5 compatibility
- instead of "return false;" use "return QDialog::eventFilter(object,
event);" to harmonize this event filter with our default behaviour
- remove orphan spaces found while editting the files
