Cleanup code using forward declarations.
...
Use misc methods of avoiding unnecesary header includes.
Replace int typedefs with int##_t from stdint.h.
Replace PRI64[xdu] with PRI[xdu]64 from inttypes.h.
Normalize QT_VERSION ifs where possible.
Resolve some indirect dependencies as direct ones.
Remove extern declarations from .cpp files.

If (for example) the new max_size must be 5 then at the end the queue.size() must be <= 5, so the exit condition must be <=, so the continuing condition must be > (and not >= as it was).

Update License in File Headers
...
I originally created a pull to replace the "COPYING" in crypter.cpp and
crypter.h, but it turned out that COPYING was actually the correct
file.
