and built-in dependencies.

Modifies a lot of `&vector[]`'s to `vector.data()`'s across all the files including tests, just the stuff that 9804 missed

`bitcoind -X -noX` ends up, unintuitively, with `X` set.
(for all boolean options X)
This result is due to the odd two-pass processing of arguments. This
patch fixes this oddity and simplifies the code at the same time.
Make sure that chainparams and logging is properly initialized. Doing
this for every test may be overkill, but this initialization is so
simple that that does not matter.
This should fix the travis issues.
- Add license headers to source files (years based on commit dates)
in `src/test` as well as `qa`
- Add `README.md` to `src/test/data` specifying MIT license

Use misc methods of avoiding unnecesary header includes.
Replace int typedefs with int##_t from stdint.h.
Replace PRI64[xdu] with PRI[xdu]64 from inttypes.h.
Normalize QT_VERSION ifs where possible.
Resolve some indirect dependencies as direct ones.
Remove extern declarations from .cpp files.
- explicitly set the default of all GetBoolArg() calls
- rework getarg_test.cpp and util_tests.cpp to cover this change
- some indentation fixes
- move macdockiconhandler.h include in bitcoin.cpp to the "our headers"
section
