Mixing on a newer version with small number of upgraded masternodes
causes mixing txes to "stuck" - old nodes refuse to relay dstxes from new ones because
they come from the same masternodes too often. Basing threshold on the total number
of up to date masternodes and not only on a newer (enabled) ones should solve this.
This however will cause mixing to fail if there are not enough masternodes
on the needed protocol version (i.e. at least GetMinMasternodePaymentsProto())
but it's better to stop mixing than having funds kind of spent yet not confirmed
for a long time.

immediately. If the same item is received from another node, an entry is
added to mapAskFor which marks the item for re-requesting in case the first
node did not respond. When the item is received from the first node,
the item was previously never removed from mapAskFor. Only the later getdata
loop in SendMessages would then gradually remove items from the map. This
is quite delayed however as the entries in mapAskFor have a timeout value.
RemoveAskFor allows to remove all entries from mapAskFor and setAskFor
when we are not interested in the item anymore (e.g. because we received
it already).

The SporkMessage is a lower-level construct which shouldn't be aware of
SporkManager. This commit moves the `sporkManager.IsSporkActive()` calls
outside the SporkMessage functions. This follows the dependency injection

There are no watchdogs and it makes no sense to limit triggers by size (invalid triggers are going to be voted down anyway).
512 bytes should be more than enough for any proposal - the only value we do not check size of is URL and with 512 bytes total limit URL has space for almost 300 bytes.
#1962 changes the minimum required confirmations for InstantSend to 2 when
running in testnet. This resulted in the quorum rank calculation to pick
blocks which are higher then the current chain tip. This commit fixes this
by calculating the added height based on nInstantSendConfirmationsRequired.
This results in the value 4 for mainnet, so no incompatibility is
introduced. On testnet and regtest, this results in 0.

The reason for this removal is that this block was a copy/paste when we spit one huge privatesend file into multiple classes/files trying to make as little changes as possible to not to break anything. If you look at CPrivateSendServer::SetState() you'll notice that these states (POOL_STATE_ERROR and POOL_STATE_SUCCESS) are illegal for masternodes anyway, so no reason to have conditions relying on that or trying to set illigal state.
